1. Sponsor. The Fitz Aesthetic Club, LLC d/b/a The Fitz Aesthetic Club, 636 Green Bay Road, Kenilworth, IL 60043 ("The Fitz" or "Sponsor").

2. Eligibility. Open to individuals who are at least 18 years old at the time of entry and are legal residents of Illinois. Employees of Sponsor and members of their immediate households are not eligible. Void where prohibited.

3. Entry period. The giveaway begins upon posting of these Official Rules on October 1, 2026 and ends Sunday, October 4, 2026 at 11:59 p.m. Central Time (the "Entry Period"). Sponsor's clock is the official timekeeper.

4. How to enter. Visit thefitzmed.com/winnetkahockey and complete the entry form. Limit one entry per person. Entries that are incomplete, duplicate, or submitted by automated means will be disqualified.

5. Prize. One winner will receive a private experience at The Fitz North Shore in Kenilworth, Illinois, for the winner and up to three guests. The experience may include skin analysis, consultations, and aesthetic or medical treatments; light food and beverages; and skincare gifts. Approximate retail value of the entire prize: $5,000. Actual value will vary based on the services provided.

6. Medical treatments. All treatments are subject to a consultation and a determination of medical appropriateness by a licensed provider and will be at the provider's discretion. The winner and each guest must complete the medical provider's standard intake and consent forms. No specific treatment is guaranteed, and the medical provider may decline or modify any treatment for any person.

7. Guests and alcohol. Guests must be at least 18 years old. Alcoholic beverages will be served only to guests aged 21 or older.

8. Scheduling and conditions. The experience must be scheduled at a mutually agreeable date and time and used by March 27, 2027. The prize is non-transferable and may not be exchanged for cash. Sponsor may substitute a prize or prize component of equal or greater value. Any portion not used by the deadline is forfeited.

9. Winner selection and notification. One winner will be selected in a random drawing from all eligible entries received during the Entry Period. The winner will be drawn and notified on or about Monday, October 5, 2026 by email and/or phone, using the contact information provided at entry. If the winner does not respond within 48 hours, is ineligible, or does not comply with these rules, an alternate winner may be selected. Odds of winning depend on the number of eligible entries received.

10. Taxes. The winner is responsible for any taxes on the prize. Sponsor may require tax information and may issue an IRS Form 1099 as required by law.

11. Release. By accepting the prize, the winner and guests release Sponsor and its affiliates, providers, and agents from claims arising from participation in the giveaway or acceptance or use of the prize, except as prohibited by law and except for claims arising from Sponsor's gross negligence or willful misconduct.

12. Publicity. Sponsor will not use the winner's name or likeness in promotional materials without the winner's separate written permission.

13. Privacy. Information collected at entry is used to administer the giveaway, as described in Sponsor's Privacy Policy. You will receive marketing email only if you opt in on the entry form, and you may unsubscribe at any time.

14. General. Sponsor may cancel, suspend, or modify the giveaway if fraud, technical failure, or another cause beyond its control affects its administration. This giveaway is not sponsored, endorsed, or administered by Winnetka Hockey Club, New Trier Hockey, or any school or league. These rules are governed by Illinois law.

15. Winner's name. To request the winner's first name and last initial, email info@thefitzmed.com within 30 days after October 5, 2026.
