After booking, you will receive an email and text confirmation. After your appointment is confirmed, all required paperwork will be posted to your Patient Portal where it can be signed with ease.
Cancellation Policy

Cancellations and rescheduling requests must be made 24 hours prior to your appointment. If you cancel your appointment less than 24 hours in advance or do not show up for your appointment, your payment method on file will be charged a $100 fee. If you arrive for your appointment more than 10 minutes late, your appointment may be canceled, and you will incur a $50 fee for appointments 30 minutes or less or a $100 fee for appointments over 30 minutes.
